Fall Creek Internal Medicine is the recipient of Oregon’s 2018 HealthInsight Quality Award for Outpatient Practices. This award recognizes excellence in performance on adult and pediatric Quality of Care Composite Measures drawn from the Oregon Data Collaborative, an all-payer claims database (commercial, Medicaid and Medicare). The Quality Award incorporates nationally endorsed clinical quality measures, scoring performance by comparing the number of people who actually received a recommended health care service with the number who should have received it. The Quality Award recognizes outpatient practices with performance ranking in the top 3 percent (at or above the 97th percentile). Fall Creek is honored to be the only clinic in Central Oregon to receive this recognition.

Comprehensive Primary Care at Fall Creek
Comprehensive Primary Care (CPC+)
Our practice is participating in CPC+, the nations largest-ever program to improve primary care. Through CPC+, our insurers will give our practice additional resources to help us better manage your care. We hope to provide you the highest quality patient-centered care.
To help us take better care of you, Medicare will start sharing some of your personal health information with us. This will help provide us with a more complete picture of your health and be better able to coordinate your care.

Your Medicare benefits aren’t changing. You still have the right to use or visit any doctor or hospital that accepts Medicare, at any time. Your doctor may continue to recommend that you see particular doctors for your specific health needs, but it’s always your choice which doctors you use or hospital you visit.
